• Declaration on Non judicial stamp paper of Rs. 100/- for owning or hiring key and critical equipment along with proofs.
• Self declaration of the Bidder that he is not black listed and owning the responsinility of uploaded on line is correct and genuine.
• Statement showing the litigation history
• Similar works completed with in the block period and experience certificates from the EE and Counter signed by SE.
• Report on the financial stainding of the Bidder such as profit/loss statements , balance sheet and auditors report obtained from CA for the past 3 years.
• Qualification certificate of key personnel.
• Annual Turn over certificate certified by CA or issued by Excecuting Authority along with balance sheet
• Experience certificate within the block period in support of quantities executed. The details of values of civil engineering works executed in the last ten years on the tender name with supporting certificates that is preceeding ten years.
• EMD Online or Challan in favour of Executive Engineer, PRI Division, Vijayawada or BG in favour of Superintending Engineer,PR Circle, Machilipatnam valid for 6 months and proof of payment
