• Annual turnover certificate duty certified by CA on the basis of balance sheet of preceding 3 years.
• Registered with ESI, EPF deptt. having jurisdiction in HP, copy of account No.
• Self-declaration regarding non-conviction, not blacklisted/debarred by any deptt. or institution.
• Experience certificate regarding providing services of similar nature, not less than 2 years.
• BOQ for outsourcing various unskilled works at ETT/IVF Lab. Palampur at Banuri HP.
