Annual Rate Contract for Microbiology Lab Chemicals at Haveri Institute

Haveri Institute of Medical Sciences, Haveri has issued a national competitive bidding e-tender for the purchase of chemicals required for the Microbiology Lab on an annual rate contract basis. • 33PERCENT Bank Solvency certificate should be produced by the Nationalized banks.Tender number must be mentioned in the Solvency certificate
• Copy of Annual Turnover Minimum Rs. 40.00 Lakhs and above attested by CA for last 3 FINANCIAL years 2022-23, 2023-24 and 2024-25.
• Copy of experience and satisfactorily work done certificate of Rs. 40.00 Lakhs for supply of chemical and consumables to the Govt Institutions in the FY 2022-23, 2023-24 and 2024-25 Do not upload work orders/supply orders
• Copy of firm prevailing trade license taken by BBMP/TMC/CMC/GP/CITY CORPORATION
• Copy of Pan Card
• Copy of the firm Manufactures or authorized dealer certificate
• Copy of the Self declaration in Rs. 100 /- bond paper should be taken in the name of Dean and Director, HIMS, Haveri ie The firm is not in Black listed and any other issues as per annexure-V. Bond Should be taken after tender call dated.
• EMD Is compulsory for all bidders and exemption for MSME with NSIC certificate holders.should upload EMD submitted copy
• GST Registration Certificate and GST-3B return from January-2026 to July-2026 Gst Clearance certificate till July -2026.
• Income Tax return and Audit Report from Chartered Accountant for Last 3 Years FY 2022-23, 2023-24 and 2024-25
• Manufacturer should Submit the MSME Certificate
• Non-Refundable DD of Rs. 5000 to be drawn in favor of The Director, HIMS, Haveri and to be uploaded in the kppp portal. The original DD must be produced at the the time of document verification.
