Construction of Urban Primary Health Center Building

This tender is for the construction of a new Urban Primary Health Center building at the Old Government Hospital site in the 8th ward of Chilakaluripet Municipality. The project is funded under the 15th FC Grant and General funds. The consignee is Chilakaluripet Municipality, located in Chilakaluripet.

• Copy of valid Registration Certificate in appropriate class as per G.Os with Government of Andhra Pradesh
• Proof of receipt towards EMD through online/ BG/ISB
• Proof of receipt towards Transaction Fee
• Details of Value of Civil Engineering works executed during the last ten financial years on the tenderers name in statement I with supporting certificates
• Details of similar works completed as prime contractor in the same name during the last ten financial years in statement II with supporting certificates
• Details of existing commitments i.e., works on hand and works for which tenders are submitted in statement IV with supporting certificates
• Annual Turnover certificate certified by CA or issued by Executing Authority along with balance sheet.
• Any other documents required as per Tender Schedule.
• Certificate in support of existing commitments.
• Copy of declaration stating that the soft copies uploaded by them are genuine
• Copy of information regarding any litigation with Govt in which the tenderer is involved.
• Copy of liquid assets or credit facilities or solvency certificate or Net worth Certificate
• Copy of PAN Card along with Proof of Submission of latest Income Tax returns
• Experience certificate within the Block Period in support of Quantities executed.
• Qualification certificate of key personnel.
• Similar Work Completed Within the Block Period experience certificate.
• GST Registration is must in Andhra Pradesh AP
• Scanned copy of declaration on critical equipment owned on non-judicial stamp paper of Rs.100/-.
